The UCSF Hand and Microvascular Surgery Service is a coordinated service that provides emergency and consultation services to the three main campuses of the UCSF School of Medicine: (1) UCSF Moffitt/Long Hospital; (2) San Francisco General Hospital; (3) UCSF/Mt. Zion Hospital Medical Center. The present responsibilities of the Hand and Microvascular Surgery Service include emergency level coverage for emergencies at UCSF Moffitt-Long Hospitals and Mt. Zion Hospital, including acute microvascular and replantation services, and consultant services for San Francisco General Hospital. Private office hours are held two afternoons a week at UCSF at the Parnassus and Mt. Zion campuses.
Hand clinics meet two half-days a week at San Francisco General Hospital and the UCSF Ambulatory Care Center. There is variable Resident exposure to these practices, with the Residents concentrated on the clinic experiences. The Fellow participates in all office hours – private and clinic. Operative cases are seen at the UCSF Ambulatory Surgery Center, which is a very efficient, modern, outpatient surgery center, on the UCSF Parnassus campus, as well as at the main operating room at UCSF Moffitt/Long. Elective and emergency patients are seen at Mt. Zion Hospital on an as-needed basis. San Francisco General Hospital has regularly scheduled hand and microvascular cases one day a week, where the service assists the Residents on complex reconstructive cases.
The model for this Fellowship is a one-on-one apprenticeship with close supervision and interaction with the Attending hand faculty. Fellowship clinical responsibilities include functioning as the primary assistant to the Attending staff during surgery and performing as junior Attending staff with supervising Residents. The Fellow will evaluate in-house consults and both private and clinic patients, and observe or consult with the Attending staff.
The weekly schedule for the Hand and Microvascular Surgery Service, which is subject to change as the need arises, is as follows:
7:30 am-3:30 pm, Moffitt Long Operating Room, block time (ACC)
8 am-12 noon, New Patient Clinic MT. Zion,
1 pm – 5 pm, SFGH Operating Room or Mt.Zion Clinic
8:30 am-2:30 pm, UCSF Operating Room, block time (ACC)
7:30 am-4:00pm, Mt.Zion Clinic 1st and 2nd Thursday, 3rd Thursday Shriner’s OR and 4th Thursday Mt.Zion Clinic
8:00 am -12:00 noon, Research Time
1:00 pm – 4:00, Problem Hnd & Upper Extremity Clinic. SFGH
Additional Schedule Notes:
- On the 1st Monday, The fellow attends Arthritis Clinic at Moffitt/Long from 8am to 12pm.
- On the 1st Tuesday, the fellow attends the Nerve injury Clinic at Moffit/Long. This clinic is held from 4:00pm to 5:30pm.
- On the 3rd Tuesday, Wednesday, Thursday of each month, the fellow is assigned to Shriners Hospital OR & Clinic.
As a clinical Fellow within a full-service academic orthopaedic department and residency program, the Fellow has the opportunity to attend the full complement of organized conferences and rounds. These include:
- Monday, 7 am: Spine conference, UCSF (weekly)
- Tuesday, 7 am: San Francisco General orthopaedic conference (weekly)
- Wednesday, 7 am: UCSF Orthopaedic Grand Rounds (weekly)
- Wednesday, 8 am: UCSF orthopaedics basic science conference (weekly)
- Thursday, 7 am: Attending rounds/orthopaedic service, UCSF
- Friday, 7 am: UCSF hand conference (weekly)
- Saturday, 8 am: Bay Area combined orthopaedic grand rounds; topic and hospitals on a rotating schedule (once a month 1st Saturday of month at UCSF.
At the weekly Friday hand conference, Residents and hand practitioners are integrated into a combined conference format, which will include both didactic lectures, informal teaching sessions, and case presentations.
The UCSF Department of Orthopaedic Surgery has an integrated multi-disciplinary orthopaedic research laboratory. Components include: the Orthopaedic Biomechanics Laboratory, the Microsurgery Laboratory, and the Molecular and Cellular Biology Laboratory. The Orthopaedic Research Laboratory is a major component in the development of the orthopaedic surgery department under the chairmanship of David S. Bradford, M.D., with Jeffrey C. Lotz, Ph.D., as Laboratory Director. Already state-of-the-art equipment, such as MTS and MMED mechanical testing machines, full computer capabilities, full histology capabilities and video imaging are in place.
The Hand and Microvascular Surgery Service is an integral part of this research initiative. There are presently two operating microscopes, a Mac computer and a printer in the Microsurgery Laboratory. Clinical microscopy training is provided in the Fellowship. Two research Fellows are active in projects and also help direct annual microsurgical laboratory training to physicians in other disciplines, (oral and maxillofacial surgery, urology, etc.) Hand research Fellows perform basic science projects in wrist and hand biomechanics, soft tissue repair, animal research, imaging studies, and any other topics that are within the capabilities of the laboratory. There is a weekly hand lab meeting, Thursday mornings from 8-8:30, with informal meetings with individual investigators on individual projects on an as-needed basis. The lab presently has major projects underway in flexor tendon healing, developing an animal model for repetitive motion injuries and chronic nerve compression, CT imaging and finite element analysis of wrist anatomy, and implant experimentation and design. The laboratory is supported by grants from UCSF, the Orthopaedic Research and Educational Foundation, the National Institutes of Health, and private donations. Clinical projects and areas of interest include: evaluation of outcomes of carpal tunnel surgery, external fixators for wrist fractures, and clinical trials of novel splinting and casting devices.
An important feature of the Combined Hand and Microvascular Surgery Fellowship is one-on-one teaching of research methodology, including formulation of a clinical or basic research question, setting up the protocol for the experiment, and producing a manuscript or a paper. All Fellows are required to complete a project prior to the completion of the Fellowship, with national meeting presentation/publication expected. Adequate time and supervision is provided to make this a realistic and integral part of the Fellowship.
