Before requesting to book the HPC, please review the HPC Operations and Booking Policy.

Next, please complete the HPC Booking Sheet- General Use, if requesting the room for meetings, presentations or workshops OR complete the HPC Booking Sheet- Research Projects if requesting the room for research or pilot projects.

Please e-mail the completed booking sheet at least one week in advance to Cindy Conti at ContiC@orthosurg.ucsf.edu and Dr. Anthony Luke at lukea@orthosurg.ucsf.edu.
