Common Questions and Answers

I’ve attended a meeting and seek my credits:
Claiming Credits – How do I get my CME Certificate for attending a conference?
Instructions for obtaining your CME Certificate will be provided during the conference. Typically you will be required to turn in your evaluation and Attendance Verification record (AVR) then your certificate will be mailed to you post course at the address you registered with. For further assistance, you may contact the CME office at 415-476-5808 or email Erin Simon at simone@orthosurg.ucsf.edu.

What if I forgot to claim my credit?
Attendance Verification Form – If you lost your original form or didn’t turn it in before the end of the course, download one here to claim your CME credit. Forms should be signed and faxed to 415-502-1795.

Duplicate Certificates
Please use our CME Certificate Form if you need a duplicate CME certificate for a prior course. UCSF maintains records for six years.
