Thank you for your interest in your son or daughter's participation in High School Athletics. Below you will find important information and dates regarding Pre-Participation Physical Exams (PPE):

 
Dates, Times, and Locations

UCSF is not currently conducting physicals at SFUSD High Schools or at the Sports Medicine Clinic. Please check back this summer for upcoming physical dates this Spring.

CURRENT OPTIONS

• 1) Contact your primary care physician.
  • *The Best Option. Ideal way to continue medical care with your family physician.
• 2) Teen Clinic at SF General Hospital.
  • Call 415-206-3877 to schedule an appointment.
  • The teen clinic has evening hours one evening a week from 1:00 PM to 8:00 PM.
  • *SF General Hospital is an ideal option for those individuals lacking or with minimal insurance.

Documents and Forms

IMPORTANT REMINDER!!!
Please make sure to have the following steps completed prior to your physical appointment.

1) Completely fill out all documents, answering questions to the best of your ability.
2) Make sure to have the athlete's Parent or Guardian sign the bottom of the document.

SFUSD Athletic Pre-Participation Physical Evaluation Document.

1) Write Athlete's Name in Blank Space.
2) Check/Mark Blank Space next to "I Authorize...".
3) Sign, Print, Date the bottom of the document.

UCSF Consent for Pre-Participation Physical Evaluation Document.

A $10 payment is appreciated at the time of service. Both check or cash are welcome.
