REGISTRATION INSTRUCTIONS
1. OBTAIN THE PROPER PAPERWORK:

- The following documents are needed:
- SFUSD Pro-Participation Examination Form (Speak with your Athletic director if your school uses a different physical form.)
- UCSF Consent for a Physical Examination Form
2. SIGN AND COMPLETE YOUR PAPERWORK:
- In order to be officially registered for thsi event, each athlete must submit the following.
- COMPLETED and SIGNED (Both athlete and guardian) SFUSD Physical Exam Paperwork. (Please complete the first page of this form completely, noting your child's past medical history)
- COMPLETED and SIGNED (Both athlete and guardian) UCSF Consent Form.
- SFUSD:
- No sign-up needed, Bring Paperwork, Show up at 8am, Wear Shorts.
- Non- SFUSD:
- Sign-up with your AD, ATC, Bring Paperwork, Show up at 8am, Wear Shorts.

IMPORTANT REMINDER!!!
Please make sure to have the following steps completed prior to your physical appointment.
- 1) Completely fill out all documents, answering questions to the best of your ability.
- 2) Make sure to have the athlete's Parent or Guardian sign the bottom of the document.
- 1) Write Athlete's Name in Blank Space.
- 2) Check/Mark Blank Space next to "I Authorize...".
- 3) Sign, Print, Date the bottom of the document.
